Removes language that froze Medicaid rates for supportive living facilities at June 30, 2018 levels and establishes new rate-setting methodology effective July 1, 2019.

Sets Medicaid rates for supportive living services at 60% of the average total nursing facility services per diem for defined geographic areas, with dementia care facilities receiving 72% instead of 60%.

Requires Medicaid rates for supportive living services to be automatically updated whenever nursing facility service per diems are updated.

Limits the delink of supportive living facility per diem rates from nursing facility per diem rates to the period of May 1, 2011 through June 30, 2019.

Takes effect immediately upon becoming law.
