Establishes a Small and Mid-sized Business Enterprise Initiative program under the Illinois Procurement Code for construction contracts and goods/services by the Department of Transportation and Capital Development Board.

Defines small and mid-sized businesses as local enterprises with gross receipts or employee counts not exceeding 1.5 times the size standards in 13 CFR Part 121, and requires businesses to operate for at least 2 years to be eligible.

Authorizes chief procurement officers to set aside construction projects valued between $3,000,000 and $10,000,000 exclusively for small and mid-sized business participation under a race and gender-neutral program.

Requires eligible businesses to perform more than 50% of work with their own workforce or small/mid-sized subcontractors, and mandates annual affidavits attesting to eligibility status.

Imposes penalties for fraudulent misrepresentation including Class C misdemeanor charges, fines of $1,000-$5,000 per offense, and potential ineligibility from future contracts up to permanent debarment.
