Creates the Choose Life Fund as a special fund in the State treasury to receive fees from Universal special license plates with Choose Life decals.

Authorizes Illinois Choose Life, Inc. to issue decals for Universal special license plates with an original issuance fee of $25 ($10 to Choose Life Fund, $15 to Secretary of State Special License Plate Fund) and renewal fee of $25 ($23 to Choose Life Fund, $2 to Secretary of State Special License Plate Fund).

Establishes that Universal special license plates may only be affixed to passenger vehicles of the first division (including motorcycles and autocycles) or motor vehicles of the second division weighing not more than 8,000 pounds, and plates expire according to the multi-year registration procedure.

Requires authorized organizations issuing decals to comply with Secretary of State rules and prohibits any decals, stickers, or materials on Universal special license plates other than those authorized by the General Assembly or registration renewal stickers.

Permits the Secretary of State to issue Universal special license plates as vanity or personalized plates and requires motorcycle and autocycle versions of authorized special registration plates.
