Creates the Youth Homelessness Prevention Subcommittee under the Governor's Cabinet on Children and Youth to develop a strategic vision for preventing homelessness among youth leaving State systems of care.

Subcommittee shall review discharge planning and procedures for youth leaving the Department of Children and Family Services, Department of Juvenile Justice, Department of Human Services' Division of Mental Health, and Department of Corrections to ensure housing stability.

Requires collection of data on housing stability for youth one year after release from State custody and creation of a plan to improve discharge policies based on findings.

Subcommittee membership includes directors of relevant State departments, four representatives from agencies serving homeless youth, advocacy organization representatives, and four youth with lived experience of homelessness.

Subcommittee must hold at least 6 meetings annually and submit interim reports every 6 months and annual reports to the Governor and General Assembly.
