Amends the Illinois Income Tax Act to create a voluntary income tax checkoff for contributions to qualified veterans home funds, effective for tax years ending on or after December 31, 2019.

Requires the Department of Revenue to print the checkoff provision on standard individual income tax forms allowing taxpayers to contribute any amount of $1 or more to one or more designated veterans home funds.

Specifies that contributions will reduce the taxpayer's refund or increase the amount of payment owed, and that failure to remit the increased payment reduces the contribution accordingly.

Defines "qualified veterans home fund" as one of four specific funds: the LaSalle Veterans Home Fund, the Anna Veterans Home Fund, the Manteno Veterans Home Fund, or the Quincy Veterans Home Fund.

Provision does not apply to amended tax returns.
