Prohibits the Department of Healthcare and Family Services from expanding care coordination programs to recipients of medical assistance who receive care at nursing homes, intermediate care facilities for persons with developmental disabilities, intellectual/developmental disability community care facilities, or supportive living facilities, unless they are enrolled in the Medicare-Medicaid Alignment Initiative Program.

Amends the Illinois Public Aid Code Section 5-30 regarding care coordination by adding a new subsection (l) that creates an exception to the Department's mandate to enroll medical assistance recipients in care coordination programs.

The prohibition applies specifically to institutional and facility-based care settings while exempting those enrolled in the Medicare-Medicaid Alignment Initiative.

Takes effect immediately upon becoming law.
