Establishes the Family and Medical Leave Act providing employees with 12 weeks of leave per calendar year.

Permits leave for personal illness, injury, or medical appointments; illness, injury, or medical appointments of specified family members (child, spouse, parent, sibling, grandparent, and others); and birth or adoption of a child under one year of age.

Allows employees to take leave intermittently and choose whether to use family and medical leave or other available leave; employers cannot require use of family and medical leave before other leave.

Requires employers to pay the full cost of health insurance coverage for employees during family and medical leave periods.

Mandates reinstatement of employees to their previous position or an equivalent position upon return from family and medical leave.
