Creates the Outdoor Rx Program Act establishing a grant program administered by the Department of Public Health to fund outdoor environmental, ecological, agricultural, and nature-based therapy programs for Illinois residents

Establishes an advisory committee of at least 7 members appointed by directors of Public Health, Natural Resources, Veterans' Affairs, and the Environmental Protection Agency, plus representatives from business and outdoor organizations

Prioritizes programs that utilize veterans for at least 50% of implementation/administration and serve participants recovering from substance abuse or physical/mental trauma

Creates the Outdoor Rx Program Fund as a special fund in the State treasury, funded through gifts, grants, and donations, with the Department authorized to require matching contributions from applicants

Requires annual reports to the General Assembly by February 1 detailing fund deposits, disbursements, grant recipients, award amounts, and financial projections
