Jackie Dominowski Kostrzewa passed away peacefully on July 13, 2020, surrounded by her children at her home.

She was a devoted mother to four children and grandmother to five grandchildren, and an active resident of the North Mayfair community on Chicago's northwest side.

Served on the North Mayfair Improvement Association Board as financial secretary and vice president, worked as a crossing guard for many years, and volunteered with the 17th Police District CAPS Program for decades.

Worked as an election judge for over 20 years and was inducted into the 2018 Chicago Senior Citizen Hall of Fame for her dedication to the North Mayfair community.

The Illinois House of Representatives expresses condolences to her family, friends, and loved ones.
