Amends the Firearm Owners Identification Card Act to require persons receiving revocation or suspension notices to surrender their card to local law enforcement within 48 hours and provide a receipt.

Requires completion of a Firearm Disposition Record form prescribed by the Department of State Police that discloses the make, model, and serial number of each firearm, storage location, and transferee information if applicable.

Mandates that any transferee sign a statement acknowledging awareness of unlawful firearm transfer laws, penalties for violations, and intent to retain possession until the original owner regains legal eligibility or a new transferee is designated.

Authorizes local law enforcement or sheriffs to petition circuit court for a search warrant if the person fails to comply, and establishes violation of these requirements as a Class A misdemeanor.

Requires the Department of State Police to provide written notice within 30 days to persons with revoked, suspended, or expired cards who have not surrendered them, with compliance required within 48 hours of receiving notice.
