Requires case manager training to include family futures planning information for persons with Alzheimer's disease or related disorders, in addition to existing training for caregivers of adults with developmental disabilities

Extends care coordination unit funding of $200 per completed Medicaid application to help eligible persons with Alzheimer's disease enroll in medical assistance benefits

Adds representatives committed to serving persons with Alzheimer's disease to the Community Care Program Advisory Committee membership requirements
