Amends the Uniform Child-Custody Jurisdiction and Enforcement Act to require parties in child-custody proceedings to disclose the child's address, residence history for the past five years, and names and addresses of persons with whom the child has lived.

Requires parties to disclose participation in prior custody or visitation proceedings, any related proceedings (including domestic violence, protective orders, termination of parental rights, and adoptions), and information about non-party individuals with physical or legal custody or visitation rights.

Establishes a continuing duty for parties to inform the court of any new proceedings in Illinois or other states that could affect the current custody proceeding.

Permits parties to omit their address or a family member's address from filed documents if disclosure would risk abuse or harm, and allows omission of domestic violence safe house addresses or addresses changed due to protective orders.

Authorizes courts to stay proceedings until required information is furnished and to examine parties under oath regarding the disclosed information and matters pertinent to the court's jurisdiction.
