Department of Human Services shall create a program to select individuals from the PUNS (Prioritization of the Urgency of Need of Services) database to receive housing and community-based support services.

By June 30, 2023, every selected individual must qualify for and receive housing unless they choose in-home services instead.

Department shall reform the community-integrated living arrangements model into a flexible housing model using a needs-based sliding scale to determine eligibility and placement for persons with developmental disabilities under the Home and Community-Based Services Waiver.

Department of Human Services, in collaboration with the Department of Healthcare and Family Services, must submit progress reports to the General Assembly starting May 31, 2020, and annually thereafter.

Implementation is contingent upon the Departments obtaining all necessary federal waivers and approvals.
