Amends the Illinois Public Aid Code to establish reimbursement rates for Children's Community-Based Health Care Centers providing nursing care to transition children from hospitals to home or other appropriate settings.

Department of Healthcare and Family Services shall reimburse eligible individuals at the lower of the center's usual and customary charge or $950 per diem for a maximum of 120 days.

Reimbursement payments are exempt from the 2.7% rate reduction otherwise required under Section 5-5e of the Public Aid Code.

Applies to centers established under the Alternative Health Care Delivery Act that provide services for family reunification and transitional care.

Takes effect immediately upon becoming law.
