Each election authority must submit an annual form to the State Board of Elections by January 31 containing information about voter outreach, poll worker training, recruitment methods, and early voting procedures.

The form must include poll worker testing documents used for certification, the number of poll workers employed, compensation levels, and details about early voting sites including locations and hours of operation.

The State Board of Elections, working with advocacy groups, shall develop the standardized form that election authorities use for submission.

The State Board of Elections must provide multiple submission options for election authorities: online, by mail, or by facsimile.

Information collected from election authorities shall be made available to the public upon request.
