Department of Children and Family Services must place locked suggestion boxes in residential treatment centers, group homes, shelters, and transitional living arrangements for youth in Department placement.

Department employees must check suggestion boxes at least once per week, and only Department employees have access to their contents.

Department must submit annual reports to the General Assembly summarizing issues and concerns submitted through suggestion boxes and the solutions provided.

Department must develop and conduct a standardized survey every 5 years in coordination with Foster Care Alumni of America Illinois Chapter, University of Illinois School of Social Work, and the Department's Statewide Youth Advisory Board to gather feedback from youth aging out of or who have transitioned out of foster care.

First survey report must be submitted to the Governor and General Assembly no later than December 1, 2021, and every 5 years thereafter, including youth experiences with State foster care services and recommendations for improvement.
