Department of Corrections must screen every person for Medicaid eligibility prior to their release on parole, mandatory supervised release, final discharge, or pardon.

Correctional facility officials shall assist eligible persons in completing Medicaid applications to ensure benefits begin as soon as possible after release, with the application including the person's post-release residence address.

If an eligible person's residence is temporary, they must notify the Department of Human Services of address changes when transitioning to permanent housing.

Forty-five days before discharge, the Department must offer uninsured persons an opportunity to apply for health care coverage including medical assistance and provide assistance with application completion.
