Establishes Medicaid rates for supportive living services at 60% of average nursing facility per diem rates for geographic areas, effective July 1, 2019, with rates updated whenever nursing facility per diems are updated.

Sets Medicaid rates for supportive living facilities specializing in dementia care at 72% of average nursing facility per diem rates instead of 60%.

Delinks the per diem rate for supportive living facility services from nursing facility services for the period May 1, 2011 through June 30, 2019.

Allows mixed-use buildings to house supportive living facilities if distinct parts are completely separate with no shared entrances, common spaces, or staffing with other services.

Takes effect immediately upon becoming law.
