Amends Section 3-5-2 of the Unified Code of Corrections (730 ILCS 5/3-5-2) concerning prisoner records maintained by the Department of Corrections.

Makes a technical grammatical correction by changing "The The" to "The" in the section text.

Requires the Department to maintain records of examination, assignment, transfer, and discipline of committed persons and grievances filed in its institutions, facilities, and programs.

Specifies that records must contain the names of persons involved, time, date, place, purpose of procedures, decisions, basis for decisions, and any review of those decisions.
