• Guardian must consider the ward's current preferences to the extent the ward can participate in decision-making when those preferences are known or reasonably ascertainable.

• Guardian decisions must conform to the ward's preferences unless the guardian reasonably believes doing so would cause substantial harm to the ward's welfare or financial interests.

• If the guardian cannot determine the ward's preferences, decisions shall be made by conforming as closely as possible to what the ward would have done or intended, considering the ward's personal, philosophical, religious, and moral beliefs.

• When the ward's wishes remain unknown after reasonable efforts or would cause substantial harm, the guardian shall decide based on the ward's best interests, weighing the reasons, benefits, risks, and alternatives to the proposed action.

• Act takes effect immediately upon becoming law.
