Prohibits any person, local government, fire department, or state agency from discharging or using Class B firefighting foam containing intentionally added PFAS for training or testing purposes, effective immediately upon enactment

Bans the manufacture, sale, and distribution of Class B firefighting foam and food packaging containing intentionally added PFAS starting January 1, 2021, with exemptions for uses required by federal law including FAA regulations and military purposes

Requires manufacturers of PFAS-containing Class B firefighting foam to notify sellers of the restrictions, recall affected products, reimburse purchasers, and register with the Illinois EPA with a $5,000 annual registration fee

Mandates sellers of firefighting personal protective equipment containing PFAS to provide written notice to purchasers at the time of sale starting January 1, 2021, with records retained for at least 3 years

Directs the Office of the State Fire Marshal to survey fire departments by January 1, 2021 on their PFAS foam inventory, usage history, and disposal methods, with a report to the General Assembly within 90 days of completion

Establishes civil penalties of up to $5,000 for first violations and $10,000 for subsequent violations, with penalties deposited into a new PFAS Program Fund
