Amends the Medical Assistance Article of the Illinois Public Aid Code regarding ambulance service payment appeals for services provided on and after December 15, 2012.

Requires ambulance service providers to establish medical necessity of transport using the patient care report and other available materials according to established criteria, eliminating the requirement for a Physician Certification Statement, Certificate of Transportation Services, or Medical Certification for Non-Emergency Ambulance form on appeal.

Broadens the appeal process to apply to all "ambulance services" as defined in the statute (including ambulances, medi-cars, service cars, and taxis) rather than limiting it to ground ambulance services.

Maintains existing requirements for Physician Certification Statements when patients are transported from facilities requiring non-emergency transportation, with standards based on Medicare guidelines and federal Centers for Medicare and Medicaid Services criteria.

Requires the Department of Healthcare and Family Services to increase base reimbursement rates for ground ambulance service providers to at least 112% of rates in effect as of June 30, 2018, effective July 1, 2018.
