Changes terminology from "qualified physician" to "qualified health care practitioner" (including advanced practice registered nurses, physician assistants, and licensed residents after one year of training) in provisions requiring certification of patient conditions and decisional capacity.

Requires surrogate decision makers to consult with the attending health care practitioner, review the patient's advance directive if available, and make good faith efforts to act consistently with the patient's known wishes (or use substituted judgment if wishes unknown) before voiding or revoking POLST forms or portable medical orders.

Mandates health care providers and facilities comply with Department of Public Health Uniform POLST forms, National POLST forms, other states' POLST Paradigm portable medical orders forms, or out-of-hospital DNR orders that have been executed by an adult and are apparent and immediately available.

Removes witness requirements for execution of Department of Public Health Uniform POLST forms.

Clarifies that POLST Paradigm portable medical orders forms include MOST, MOLST, POST, and POLST forms authorized by any U.S. state or territory.
