IL HB0009

Removes requirement for medical professional declaration to change sex designation on birth certificates; allows individuals to request sex designation change (male, female, or X) through signed statement attesting to gender identity or intersex condition.

Establishes fee waiver for new birth certificates for individuals released from Department of Corrections or Department of Juvenile Justice with proper verification form; limits to one waiver per person.

Waives birth record fees for homeless persons verified by human services agencies, homeless liaisons, or attorneys; limits to 4 records annually per homeless individual.

Waives birth record fees for youth in care and former youth in care under age 27; limits to 4 records annually per person.

Creates new Section 25.6 waiving birth record fees for persons residing in domestic violence shelters; limits to 4 records annually and subjects false verification to $100 penalty.

Takes effect July 1, 2023.
