Creates the Mental Health Income Tax Checkoff Fund as a special fund in the State treasury.

Allows Illinois taxpayers to voluntarily contribute to the Mental Health Income Tax Checkoff Fund on their state income tax returns beginning with taxable years ending on or after December 31, 2021, with a minimum contribution of $1.

Contributions reduce the taxpayer's refund or increase the amount of payment required with their return.

Directs the Department of Public Health to use Fund moneys to make grants to mental health service providers in the State.

Takes effect immediately upon becoming law.
