Creates the South Suburban Trauma Center Fund as a special fund in the State treasury to receive revenue from toll collection and appropriated moneys.

Imposes a $1 surcharge at toll plaza 47 from July 1, 2021 through June 30, 2031, with 20% of revenue deposited into the South Suburban Trauma Center Fund.

Requires the Health Facilities and Services Review Board, in consultation with the Department of Public Health, to designate a trauma center location on the border between Calumet City and the Village of Dolton.

Tasks the Board with selecting a healthcare provider to operate the trauma center, entering into an operational agreement, and determining revenue sources needed for maintenance.

Repeals the toll surcharge provision on January 1, 2032.
