Requires the Department of Healthcare and Family Services to cover perinatal doula services and evidence-based home visiting services under Illinois Medicaid for eligible individuals.

Defines perinatal doula services as regular visits from prenatal period through up to 12 months postpartum, including continuous support during labor and delivery, to support healthy pregnancies and positive birth outcomes.

Permits perinatal doula services to be embedded within existing programs such as evidence-based home visiting, with weekly prenatal visits, continuous labor and delivery support, and postpartum coverage up to 12 months.

Directs the Department to adopt implementing rules in consultation with doula program experts, doula training providers, practicing doulas, home visiting experts, and relevant state agencies to establish provider credentials and reimbursement rates, with biennial rate assessments.

Requires the Department to seek necessary state plan amendments or federal waivers to implement the program and secure federal Medicaid funding for these services, effective July 1, 2022.
