Requires applicants to be Illinois residents with a disability, proof of disability from another agency, income not exceeding 200% of area median income, and either homeownership or landlord permission for modifications.

Mandates thorough home assessments by trained center for independent living staff, with a Home Modification Subcommittee providing oversight for first 3 requests from each center and any requests exceeding $35,000.

Requires centers to obtain competitive bids from at least 2 licensed, insured contractors and select the lowest responsible bidder; allows the lead agency a 3% administrative fee on all program funds.
