IL HB1854

Adds Section 3-911 to the Mental Health and Developmental Disabilities Code requiring the Department of Human Services to provide written aftercare plans to recipients prior to discharge from inpatient facilities.

Aftercare plan must include illness nature and follow-up requirements, medications with side effects and dosage schedules, informed consent forms, expected recovery course, relevant treatment recommendations, and referrals to medical and mental health providers.

Plan applies to recipients admitted voluntarily or involuntarily and must be provided to the recipient, conservator, guardian, or other legally authorized representative.

Recipients may designate another person to receive a copy of the aftercare plan.
