Creates the Ronald McDonald House Charities Fund as a special fund in the State treasury to make grants to Ronald McDonald House Charities for services in Illinois.

Requires the Department of Revenue to print a checkoff provision on individual income tax forms for taxable years ending December 31, 2021 and later, allowing taxpayers to contribute any amount of $1 or more to the Fund.

Contributions to the Ronald McDonald House Charities Fund reduce the taxpayer's refund or increase the amount of payment owed with the return.

Does not apply to amended returns, and failure to remit any increased payment reduces the contribution accordingly.

Takes effect immediately upon becoming law.
