Requires the Secretary of State to issue a Voter Identification Card to registered voters who lack acceptable photo identification, containing the voter's name, signature, photograph, state seal, and current residence address.

Establishes acceptable forms of photo ID for voting as: Illinois Driver's License, State Identification Card, Disabled Person ID Card, Senior Citizen ID Card, FOID Card, U.S. Passport with current address, or any other government-issued ID with voter's name, current photograph, and current address.

Mandates that voters present either a government-issued photo ID or Voter Identification Card when voting in person, by early voting, or when casting provisional ballots.

Exempts indigent voters (income at 125% or less of federal poverty guidelines) and those with religious objections to being photographed from ID requirements, allowing them to vote a provisional ballot instead.

Requires presentation of documentation including photo identity, date of birth, voter registration proof, and residence address to obtain a Voter Identification Card, with flexible alternatives like birth certificates, marriage licenses, tax returns, and utility bills accepted.
