Changes county clerk authority from "may" to "shall" require issuance of death record certifications from the electronic reporting system and cancellation of deceased persons' voter registrations within the preceding month.

Requires county coroners, medical examiners, physicians, or other individuals certifying deaths under the Vital Records Act to transmit certified records to the county clerk within 7 days after death.

Mandates county clerk and coroner to submit quarterly reports to the affiliated county board certifying full compliance with these provisions and accuracy of voter rolls.

Allows any person to request copies of the required reports and permits filing suit for injunction or declaratory relief if the county clerk fails to provide an accurate report within 5 business days, or 48 hours if within 30 days before an election.

Establishes civil penalties of $2,500 to $5,000 per occurrence for willful and intentional non-compliance, plus up to $1,000 per day for continued violations after 30 days, and awards reasonable attorney's fees and costs to prevailing parties.
