Amends the Small Wireless Facilities Deployment Act to add a definition for "source of radio frequency emissions" and permit information requirements to be provided to the Illinois Environmental Protection Agency (IEPA).

Allows the IEPA or local authorities to deny permits if providers fail to provide sufficient evidence that increased radio frequency emissions are needed, and requires credible information on fiber optic cable availability and coverage deficiencies within the past 12 months.

Removes the previous exemption that prohibited local authorities from regulating small wireless facilities located in interior structures or on campuses, stadiums, or athletic facilities not owned by the authority.

Establishes new requirements for radio frequency emissions monitoring, recordkeeping, and setbacks: requires a minimum 200-foot setback from residences, schools, childcare facilities, and hospitals until the IEPA and Illinois Pollution Control Board adopt permanent setback rules within specified timeframes.

Mandates written notice to residents and property owners within 250 feet of proposed radio frequency emission sources at least 90 days before construction and at least 10 days before energizing the source.
