Illinois State Police shall establish and maintain a database of persons diagnosed with communication disabilities or disabilities that impair communication.

Department of Human Services shall develop a verification form for voluntary registration that includes name, identification numbers, vehicle license plates, and physician/psychiatrist/clinical psychologist certification of the disability.

Persons 18 years or older with such disabilities may self-register; parents or guardians may register minor children or wards by submitting the completed verification form to Illinois State Police.

The database shall be available to state and local law enforcement through the Law Enforcement Agencies Data System (LEADS) but is not subject to public records requests under the Freedom of Information Act.

Registered individuals or their parents/guardians may request removal from the database at any time by submitting a form indicating the desire to be removed, and Illinois State Police shall immediately remove them upon receipt of a properly completed request.
