Requires voters to present either a government-issued photo identification card (as defined in Section 3-8) or a Voter Identification Card issued by the Secretary of State when voting in person or by early ballot.

Creates a new Voter Identification Card issued by the Secretary of State for registered voters who lack acceptable photo ID, containing voter's name, signature, photograph, state seal, and current residence address.

Defines acceptable forms of photo identification for voting as Illinois Driver's License, State Identification Card, Disabled Person ID, Senior Citizen ID, FOID Card, U.S. Passport, or other government-issued identification with voter's name, current photo, and address.

Provides exemptions for voters who are indigent (income at 125% or less of federal poverty guidelines) or have religious objections to being photographed; these voters may cast provisional ballots and sign affidavits stating their exemption.

Requires presentation of documentation including photo or non-photo identity documents, proof of voter registration, and proof of current address to obtain a Voter Identification Card.
