Creates a Voter Identification Card issued by the Secretary of State for registered voters without acceptable photo ID, which includes the voter's name, signature, photograph, state seal, and current residence address.

Requires all voters to present either a government-issued photo identification card (such as Illinois Driver's License, passport, or FOID card) or a Voter Identification Card before receiving a ballot in-person or by early voting.

Exempts indigent voters (income at or below 125% of federal poverty guidelines) and voters with religious objections to photography from the photo ID requirement; these voters may cast a provisional ballot and sign an affidavit.

Establishes new procedures for vote-by-mail ballot signature verification using 3-judge panels that must unanimously reject ballots; requires election authorities to notify rejected voters within 2 days and allow them to submit a confirming statement or request a new ballot.

Modifies provisional ballot procedures to allow voters to provide photo identification within 10 days if initially rejected for lacking acceptable ID, and extends the deadline for submitting required documentation from 7 to 10 days in such cases.
