Creates the Choose Life Fund as a special fund in the State treasury to receive fees from Universal special license plates with Choose Life decals.

Authorizes Illinois Choose Life, Inc. to issue decals for Universal special license plates under the Illinois Vehicle Code framework for special license plates.

Sets fees for Choose Life license plates at $25 for original issuance ($10 to Choose Life Fund, $15 to Secretary of State Special License Plate Fund) and $25 for renewal ($23 to Choose Life Fund, $2 to Secretary of State Special License Plate Fund).

Allows Universal special license plates to be issued to passenger vehicles of the first division and motor vehicles of the second division weighing not more than 8,000 pounds, with plates expiring under the multi-year registration procedure.

Requires any organization receiving fees from special license plate sales to comply with Charitable Trust Act and Solicitation for Charity Act requirements and provide annual compliance letters from the Attorney General.
