Department must contract with a health care organization to provide patient-centered services including patient navigators, tailored preventive health care plans, telehealth consultants, and a patient portal

Required services include an online "virtual" health hub with wellness resources, community health centers, mobile preventive health stations and kiosks for "health deserts," and call centers to coordinate with medical homes

Implementation required within 6 months of the Act's effective date
