Department of Corrections must provide health care coverage application assistance to uninsured individuals 45 days before discharge, including medical assistance under Illinois Public Aid Code.

Department must provide information about obtaining Illinois Identification Cards and voter registration, including opportunity to participate in automatic voter registration program.

Department shall provide discharged persons with information about job listings, housing availability, and directory of elected State and local officials in their intended residence county and municipality.

Department must screen all persons for Medicaid eligibility prior to release and assist eligible individuals in completing applications to begin benefits immediately upon discharge.

Department shall provide wrongfully imprisoned persons with necessary documents before discharge and verify released persons' identity through birth certificates, social security cards, or other authorized documents.
