Amends the Humane Care for Animals Act to require the Department of Agriculture to establish an animal sourcing database tracking the origin of animals forfeited to animal shelters in Illinois.

Database must record each animal's location of origin, health information including vaccination compliance and other state health requirements, and any other relevant information determined by the Department.

Requires the Department to establish a certification board to oversee and maintain the database processes.

Authorizes the Department to adopt rules necessary to implement the database requirements.
