• Illinois State Police shall create and maintain a voluntary database allowing persons to apply to be barred from purchasing firearms through a notarized application with no reason required.

• Persons remain on the database until submitting a notarized withdrawal application, which automatically becomes effective 7 days after receipt by Illinois State Police.

• Database information cannot be used to deny Firearm Owner's Identification Card applications, confiscate firearms, require relinquishment of firearms, or for any purpose other than administering this provision.

• Illinois State Police must check the voluntary database during background checks for firearm purchases, alongside existing criminal history and mental health records.

• Voluntary registration on the database becomes a ground for denying or revoking a Firearm Owner's Identification Card.
