Requires restaurants to include one of six approved beverages by default with children's meals: water, sparkling water, flavored water, nonfat or 1% milk (130 calories max), nondairy milk alternatives (130 calories max, meeting National School Lunch Program standards), or 100% fruit/vegetable juice with no added sweeteners (maximum 8 ounces)

Allows customers to request alternative beverages not on the approved list upon request

Mandates that only approved default beverages be listed or advertised on restaurant menus and in-store advertisements for children's meals

Establishes enforcement through local health department inspections with graduated penalties: warning for first offense, $25 civil penalty for second offense, and $100 civil penalty for third or subsequent offenses

Authorizes the Department of Public Health to adopt rules for implementation and enforcement of the requirements
