Establishes the Supported Decision-Making Agreement Act as a less-restrictive alternative to guardianship for adults with intellectual and developmental disabilities who need assistance with daily living decisions

Allows adults with disabilities to designate a supporter to help them understand information, access records, and communicate decisions related to medical care, finances, housing, employment, and education without the supporter making decisions on their behalf

Disqualifies certain individuals from serving as supporters, including employers/employees of the principal (unless immediate family), paid service providers, those on the Health Care Worker Registry as ineligible, and individuals convicted of sex offenses, aggravated assault, fraud, theft, forgery, or extortion

Requires the Guardianship and Advocacy Commission to develop training materials and sample agreements for both principals and supporters, and to post these resources along with public awareness materials on its website

Agreements must be signed by both parties in the presence of two adult witnesses and can be revoked at any time by the principal or terminated automatically if abuse/neglect is substantiated or a restraining order is issued against the supporter
