Requires the Department of Children and Family Services to conduct exit interviews with foster children age 5 and older within 5 days of removal from a foster home, unless clinically contraindicated.

Exit interviews must be conducted by a caseworker, mental health provider, or clinician and must document the child's experience regarding basic needs, safety, comfort, access to professionals, and inclusion in family activities.

Interview documentation must be maintained in the foster parent's licensing file, the child's case file, included in the service plan, and provided to the child's guardian ad litem and attorney.

Adds "quality of care concerns applicant" definition to include foster care license applicants or household members who requested a youth's removal 5 or more times, and requires such applicants to submit preliminary applications before licensure.

Department must develop procedures and interview forms by January 1, 2023, and post quarterly reports on its webpage summarizing exit interview details beginning July 1, 2023.
