Allows agents to present electronic devices displaying electronic copies of executed health care power of attorney forms as valid proof of authorization, in addition to hard copies.

Permits electronic signatures and computer-generated signature codes to satisfy execution requirements for health care powers of attorney, complying with the Uniform Electronic Transactions Act and Hospital Licensing Act standards.

Clarifies that health care providers and emergency medical services personnel cannot refuse to recognize a health care agency when an agent presents an electronic copy on a device.

Requires entities providing statutory health care power of attorney forms to post information on their websites for 2 years regarding the amendments made by this act.

Changes formatting in the statutory short form notice from "on-line" to "online" to reflect current language conventions.
