Creates a health benefits easy enrollment program requiring the Illinois Department of Revenue to print a provision on individual income tax returns starting in tax year 2022 allowing taxpayers to request health insurance eligibility information and authorize income data sharing with the State health benefits exchange.

Requires the Department of Healthcare and Family Services and Department of Insurance to provide form letters describing health insurance enrollment options to the Department by June 1 and October 1 each year, unless a State health benefits exchange is operational.

Mandates the Department send enrollment information letters to interested taxpayers by July 1 and November 1 each year until the State health benefits exchange becomes operational.

Requires any State health benefits exchange marketplace to interface with the Department's tax system and authorizes the Director to share individual income tax information with the exchange for eligible taxpayers to assess potential eligibility for health insurance premium tax credits and Medicaid.

Modifies Section 917 of the Illinois Income Tax Act to allow the Director to share individual income tax information with the State health benefits exchange when authorized by the taxpayer under the new Section 513 enrollment program.
