Creates the Ronald McDonald House Charities Fund as a special fund in the Illinois State treasury, with appropriated moneys to be used for grants to Ronald McDonald House Charities for services in Illinois.

Requires the Illinois Department of Revenue to print a checkoff provision on individual income tax forms for taxable years ending on or after December 31, 2021, allowing taxpayers to contribute a minimum of $1 to the Ronald McDonald House Charities Fund.

Contributions to the fund reduce the taxpayer's refund or increase the amount of payment owed, with any unpaid increased payment reducing the contribution amount accordingly.

Checkoff provision does not apply to amended tax returns.

Act takes effect upon becoming law.
