Requires provider and hospital reimbursements for immunizations under the Healthy Kids Program to be no lower than Medicare reimbursement rates, effective January 1, 2022.

Changes immunization data reporting from optional ("may") to mandatory ("shall") for health care providers, physicians' designees, and pharmacists' designees to report data to the immunization data registry.

Removes the ability for patients or parents/guardians to opt out of immunization data registry reporting through written exemption forms, while preserving the right to request data removal after reporting.

Requires written materials and exemption forms to clearly state that health care providers "shall" report immunization data to the Department of Public Health, rather than stating they "may" report such data.
