Requires persons whose Firearm Owner's Identification Card is revoked to surrender the card to local law enforcement within 48 hours and complete a Firearm Disposition Record disclosing the make, model, and serial number of each firearm, its storage location, and any transferee information.

Makes violation of the revocation surrender requirements a Class A misdemeanor, but elevates it to a Class 2 felony if the defendant has been previously admonished by the court regarding inability to own or possess firearms.

Authorizes Illinois State Police to send a second notice to local law enforcement if a Firearm Disposition Record is not received within 5 business days, and permits Illinois State Police to request an arrest warrant if compliance is not achieved within 5 days of the second notice.

Requires defendants pleading guilty to felonies, domestic battery, aggravated domestic battery, or other firearm-prohibiting offenses to sign a written acknowledgment stating whether they currently possess firearms, confirming their understanding of the firearm prohibition, and authorizing confiscation by local law enforcement.

Mandates Illinois State Police to provide written notice of these requirements to all sheriffs and law enforcement agencies within 30 days of enactment and annually by January 31 thereafter.
