Creates the Illinois Harvest to Homes Pilot Program for 5 years, allowing eligible entities to receive funding to provide agricultural surplus to communities in need.

Department of Agriculture must adopt implementing rules within 90 days, with an advisory council comprised of representatives from environmental nonprofits, farmer advocacy organizations, specialty crop/sustainable agriculture groups, and food banks from different regions.

Eligible funding includes reimbursement to growers for picking and processing surplus, transportation costs, food bank storage and distribution costs, facility upgrade grants, marketing activities, and program administration.

Department of Agriculture must report to the General Assembly after 3 years and 5 years documenting the number of people served and implementation details, in consultation with the Department of Human Services Commission to End Hunger.

Act repeals on July 1, 2027, and takes effect immediately upon enactment.
